How do I change what is shared, and does it take effect straight away?

In Who Can See What, per person and per category. Changes apply from that moment on, nobody is notified, and you never have to explain yourself.

Open Who Can See What, find the person, and flip the switch for the thing in question. It applies from that moment on.

Two views: Bundled for quick choices, Detailed when you want to be precise about one category.

Yes, straight away

Turn something off and it stops being visible from then on. There is no delay, no overnight sync, and no confirmation step where somebody else has to agree.

What it does not do is un-see

If your son looked at your blood pressure last week and you switch it off today, he cannot see it any more, but he obviously still remembers last week.

Worth saying plainly, because "private from now on" is what a switch can honestly promise and "as though you never saw it" is not.

Nobody is told

No notification goes out when you change a setting. Your family will not get a message saying you have stopped sharing something, and there is no list they can check.

This is on purpose. A switch that announced itself would make every change an argument, and then nobody would change anything.

You do not owe anyone a reason

It is your information. You can share your medications with a caregiver and not with your daughter, or share everything for a month while you are unwell and then close it again.

The point of the feature is to make sharing easy when you want it, and stopping easy when you do not.
